About the Role

Alignmenthealthcare seeks a dedicated Bilingual Spanish Health Coach and Educator to support members in Las Vegas, Nevada through direct, face-to-face health coaching and educational services. You'll make home visits to help members understand their health conditions, develop sustainable wellness strategies, and take active steps toward better health outcomes. This full-time position is ideal for someone who speaks fluent Spanish and English, has a passion for community health, and thrives in a flexible, member-centered environment.

Core Duties

• Conduct home visits to assess members' health status, needs, and goals
• Provide health coaching and disease management education tailored to individual and family circumstances
• Help members set realistic health objectives and develop action plans
• Educate members on medication management, nutrition, exercise, and preventive care
• Communicate findings and progress updates to the care management team
• Document interactions, assessments, and outcomes in accordance with company policies and regulatory requirements
• Build trusting relationships with members to encourage engagement in their health journey

Skills & Qualifications

• Fluency in Spanish and English (spoken and written)
• Experience in health coaching, patient education, nursing, social work, or a related health field
• Strong interpersonal and communication skills with diverse populations
• Ability to travel safely throughout the Las Vegas area for home visits
• Reliable transportation and valid driver's license
• Knowledge of health coaching best practices and chronic disease management principles
• Comfortable documenting health information digitally and adhering to confidentiality standards
