As a full-time Direct Support Professional, you'll provide daily assistance to individuals with disabilities, helping them live independently while building the relationships and skills needed to thrive in their community.

Rise Family operates residential group homes, day programs, employment services, managed care, and community support across Arizona. We're looking for a compassionate, reliable caregiver to join our team and make a direct impact on the lives of the people we serve. Your role will be essential to creating opportunities for individuals with disabilities to participate fully in their communities and reach their personal goals.
What You'll Do
- Provide hands-on personal care assistance with activities of daily living, including hygiene, dressing, grooming, and meal preparation
- Accompany individuals on community outings, transportation, and social activities
- Support medication reminders and health management as directed by supervisors
- Document client progress and behavioral observations in required records and incident reports
- Help individuals develop life and employment skills through encouragement and practical guidance
- Respond to behavioral or medical concerns and contact supervisors when needed
- Maintain a safe, clean, and respectful home environment
- Build meaningful relationships and provide emotional support with respect and dignity
What We're Looking For
- High school diploma or GED equivalent
- Valid Arizona driver's license with reliable transportation
- Ability to pass background clearance and health screening
- Capability to assist with lifting and transfers safely
- Strong communication skills and comfort working with diverse populations
- Patience, empathy, and genuine commitment to supporting people with disabilities
- Reliability and willingness to follow protocols and training
