As a Community Support Specialist serving adults, you'll be an integral part of our integrated care model, working alongside professionals dedicated to advancing recovery and independence. You'll deliver direct, person-centered assistance that helps adults build the skills and connections they need to thrive in their own communities.
What You'll Do
- Work with clients to establish, plan, and accomplish meaningful recovery and wellness objectives
- Provide support across multiple settings—clients' homes, office-based sessions, and community locations
- Coach clients in practical daily living abilities that support independence and self-sufficiency
- Connect clients with community resources, benefits, and services they need and qualify for
- Facilitate relationship-building and social connection to strengthen clients' support networks
- Provide regular transportation and driving support as an essential component of community-based care delivery
