About the Role

Working within a permanent supportive housing framework, you'll guide clients across all stages of their housing journey—from street situations through stable permanent placement. Your core mission is intensive case management rooted in client strengths and delivered with a commitment to racial and social justice. You'll combine direct service with systems-level advocacy, supporting individuals experiencing homelessness and chronic health or mental health conditions toward lasting housing stability and improved quality of life.

What You'll Do

  • Meet with clients to assess individual needs and collaboratively develop Individualized Service Plans (ISP) addressing housing, income, health, mental wellness, substance use, finances, employment, and social support.
  • Connect clients to community resources, services, and benefits aligned with their goals, providing ongoing navigation support.
  • Administer clinical assessments (including VI-SPDAT) and document findings to guide care planning.
  • Review client progress at 90-day intervals and regularly thereafter, adjusting strategies to support goal achievement.
  • Maintain accurate, timely documentation and secure files in compliance with HIPAA and program policies.
  • Travel to client residences and community sites as required to deliver services and coordinate care.

Qualifications and Experience
