Case Manager – LifeSet

Catholic Charities Serving Central Washington is hiring a Case Manager to support young people transitioning to adulthood in Manito, Spokane County. This full-time Case Manager position provides hands-on guidance to help foster care youth build independent living skills, secure stable housing, continue their education, and establish careers.

The Opportunity

Young people aging out of care face a critical crossroads. As a Case Manager with Catholic Charities Serving Central Washington, you'll be the steady presence that helps them navigate that transition. Your work directly shapes whether our clients build the foundation for a secure future—a role that combines case management expertise with genuine mentorship in one of our community's most vital service areas.

Day-to-Day Responsibilities

• Assess individual strengths, barriers, and goals to develop personalized transition plans
• Support clients in locating and maintaining safe, suitable housing appropriate to their circumstances
• Help young people enroll in and successfully complete education or job training programs
• Teach practical independent living skills including budgeting, meal preparation, time management, and household management
• Assist clients in job searching, resume building, interview preparation, and employment retention
• Connect clients with community resources including food assistance, healthcare, transportation, and mental health services
• Document progress, maintain detailed case notes, and coordinate with other service providers and family members when appropriate
• Attend team meetings and participate in continuous professional development
