Position Summary

The Behavioral Health Social Worker, PRN role at SIH places you at the center of patient-centered care at Harrisburg Medical Center. What You'll Do

• Complete thorough psychosocial assessments to identify strengths, risks, and treatment goals for patients seeking behavioral health services.
• Design and update individualized care plans in partnership with the care team, patients, and families.
• Deliver short-term counseling, crisis intervention, and evidence-based support that promotes stability and coping skills.
• Coordinate seamless transitions across levels of care, including referral and discharge planning.
• Link patients with community resources, housing supports, and after-care options that sustain progress after discharge.
• Document assessments, interventions, and outcomes thoroughly while maintaining confidentiality and regulatory compliance.
• Participate in multidisciplinary rounds and contribute social-work insights that improve overall treatment quality.

What You Bring

• Master’s degree in Social Work or a closely related behavioral health field.
• Active state licensure (LMSW, LCSW, or equivalent) permitting independent practice in the required setting.
• Prior experience providing social work services within behavioral health or mental health environments.
• Demonstrated skill in crisis assessment, motivational interviewing, and trauma-informed approaches.
• Strong ability to communicate clearly with patients, families, and interdisciplinary teams under time-sensitive conditions.
• Comfort navigating electronic health records and completing timely, accurate clinical documentation.
