LPN – Remote

About This Opportunity

We are representing a confidential healthcare organization seeking a Licensed Practical Nurse for a remote position. This is an excellent opportunity for an experienced LPN who values work-from-home flexibility while delivering quality patient care through telehealth and care coordination.

Location: Remote (must reside in Central Pennsylvania)
Employment Type: Full-time or Part-time available

Responsibilities

• Provide professional nursing care for patients in Transition of Care (TOC) programs under clinical supervision
• Manage triage communications between providers, patients, and office staff via telephone and electronic systems
• Update and maintain accurate patient electronic health records and ensure appropriate provider notifications
• Perform general nursing assessments and reinforce prescribed medication regimens and treatments
• Observe, record, and report patient conditions and reactions to medications and treatments to physicians
• Coordinate patient care with other providers, case managers, and ancillary services
• Screen patients for appropriate information and instruct in sample collection and testing procedures
• Respond to and direct incoming phone calls; provide medication and treatment instructions to patients and families
• Schedule appointments with priority given to emergency situations
• Participate in patient education programs and self-care instruction
• Ensure quality and completeness of clinical documentation for outcome reporting
• Educate healthcare team members on an ongoing basis
• Maintain strict patient confidentiality and HIPAA compliance

What We're Looking For

• Graduate of an accredited school of nursing
• Valid, current Pennsylvania state license to practice as a Licensed Practical Nurse
• Previous experience in a clinic setting preferred
• Strong knowledge of nursing principles, examination procedures, medications, and patient education
• Demonstrated ability to make rapid, clinically accurate decisions based on evidence-based practice
• Basic computer skills and proficiency with electronic health records
• Experience integrating professional nursing theory into telehealth and remote nursing practice
• Excellent written, verbal, telephone, and interpersonal communication skills
• Strong time management and task prioritization abilities
• Ability to react calmly and effectively in emergency situations
• Confident, personable, self-motivated, and able to function with tact and poise in all situations
• Capability to establish and maintain effective working relationships with patients, medical staff, and colleagues
