The Opportunity

As a Support Coordinator, you'll play a vital role in helping individuals with disabilities live independently and participate fully in their communities. Working 27 hours per week, you'll combine direct personal support with assistance in managing household operations and facilitating community engagement. This position is perfect for someone committed to making a real difference in people's daily lives while maintaining a flexible, part-time schedule in the Groat Estate neighbourhood.

Day-to-Day Responsibilities

• Provide direct support to individuals with disabilities in their homes, assisting with daily living activities as needed
• Help maintain safe, clean, and organised living environments within assigned residences
• Support meal planning, food preparation, and nutritional needs
• Facilitate community outings and social activities that promote independence and inclusion
• Document activities and progress in accordance with established care plans
• Communicate clearly with colleagues and supervisors about resident needs and any concerns
• Support individuals in maintaining personal hygiene and grooming routines
• Assist with errands, appointments, and community connections
