About This Opportunity
We are representing a national healthcare organization seeking a Care Manager to join their Kansas Foster Care team. This field-based role focuses on complex care management for members with mental and behavioral health needs, working directly in communities throughout northwest Kansas.
The position is Monday through Friday, primarily 8:00 a.m. to 5:00 p.m., with occasional evening flexibility based on member and family needs. No regular weekend hours are required. This role requires up to 75% travel throughout the assigned region for home and community visits. Candidates located in or near Ellis, Russell, Trego, or Rooks counties are strongly preferred.
Responsibilities
- Develop, assess, and facilitate complex care management activities for members with mental and behavioral health needs to achieve high-quality, cost-effective healthcare outcomes
- Evaluate member needs through phone and in-home visits, recommending and facilitating personalized care plans that address behavioral health and social determinant needs
- Perform telephonic, digital, home, and community site visits to assess member needs and collaborate with available resources
- Create ongoing care plans for high-acuity members, identifying necessary providers, specialists, and community resources including mental health and substance use disorder services
- Coordinate between members, families, caregivers, community resources, and care provider teams to ensure accessible services
- Monitor care plans and member outcomes for changes in treatment, side effects, complications, and clinical symptoms, adjusting recommendations as needed
- Facilitate care coordination with appropriate providers and specialists to ensure timely access to needed care
- Collect, document, and maintain member information in compliance with state, federal, and third-party payer regulations
- Provide education to members and families on procedures, treatment options, referrals, and healthcare benefits
- Deliver feedback to leadership on opportunities to improve care quality and delivery
What We're Looking For
- Master's degree in Behavioral Health or Social Work, OR degree from an Accredited School of Nursing
- 2–4 years of related experience in care management or behavioral health
- Current licensure as a Licensed Master's Behavioral Health Professional (LCSW, LMSW, LMFT, LMHC, or LPC) OR active RN license with behavioral health experience
- Strong assessment and care coordination skills for complex behavioral health cases
- Ability to work independently in a field-based environment with significant travel
- Excellent communication skills for working with members, families, and multidisciplinary care teams
- Knowledge of mental health, substance use disorder resources, and community-based services
- Flexibility for occasional evening availability based on member needs
- Reliable transportation for regional travel up to 75% of the time
