Remote Care Coordinator (Pacific Time)

About This Opportunity

Our talent team is representing a confidential healthcare organization focused on kidney care delivery. We're seeking a Remote Care Coordinator to support chronic care management services for patients with complex conditions including chronic kidney disease, hypertension, diabetes, and cardiovascular disease.

This is a fully remote position requiring Pacific Time zone hours (8:00am – 5:00pm PT). The role is outreach-driven and operationally focused, utilizing SMS as the primary patient engagement channel with phone support as needed.

Responsibilities

• Conduct high-volume text and telephonic outreach to enrolled chronic care management patients on a weekly basis
• Support patient enrollment, onboarding, and eligibility verification based on program and payer guidelines
• Ensure patient consent documentation is completed prior to outreach and maintain proactive engagement
• Achieve required engagement time standards aligned with Medicare CCM billing requirements
• Reinforce provider-directed care plans during patient interactions and identify barriers to adherence including medication access, transportation, or social needs
• Document patient interactions accurately within CCM platforms and EHR systems with audit-ready standards
• Track engagement time consistent with Medicare CCM billing compliance requirements
• Facilitate patient access to referring providers and clinical staff while coordinating care transitions following hospitalizations or ED visits
• Coordinate referrals and follow-up appointments, escalating symptom concerns or medication issues to licensed clinical staff
• Manage patient messaging workflows and collaborate with remote clinical and operational teams
• Maintain HIPAA and PHI compliance in all documentation and communication

What We're Looking For

Required Qualifications:

• Certified Medical Assistant certification
• Bilingual fluency in Spanish and English
• 1–3 years of healthcare experience
• Strong clinical assessment, communication, and documentation skills
• Ability to work comfortably in two systems (EHR and CCM platform) simultaneously
• Capability to manage a structured, metric-driven workflow with reliability
• Intermediate proficiency with MS Office Suite including Outlook and Teams
• Home internet with minimum 25 Mbps download and 10 Mbps upload speeds (cable, fiber, or DSL hardwired connection recommended)
• Dedicated workspace free from ongoing interruptions to protect patient PHI/HIPAA information

Strongly Preferred:

• Experience supporting nephrology or chronic kidney disease patient populations
• Background in dialysis patient coordination, medication adherence monitoring, or kidney transplant/specialty referrals
• Prior work in virtual care, remote nursing, or telehealth programs
• Familiarity with CCM regulatory requirements
• Patient education and motivational interviewing skills
