About This Opportunity

We are representing a confidential healthcare organization seeking a Senior Care Manager (RN) to support high-risk members through comprehensive assessments, individualized care planning, and complex caseload management. This is a Monday–Friday role with standard business hours and flexibility available after training.

Candidates must reside in North Carolina and hold an active NC RN license.

Responsibilities

  • Assess, plan, and implement complex care management activities to enable quality, cost-effective healthcare outcomes for members
  • Develop personalized care plans and service plans, addressing issues and educating members and families on available services and benefit options
  • Collaborate with providers to identify specialists and community resources needed to address unmet member needs
  • Coordinate between members, families, caregivers, and provider teams to ensure adequate person-centered care
  • Monitor care plans and member status, including changes in condition and progress toward goals; revise plans as necessary
  • Identify problems and barriers for care management and recommend appropriate interventions for escalated cases
  • Review member data to identify trends and improve operating performance and quality care in accordance with regulations
  • Perform telephonic, digital, home, or site outreach to assess member needs and coordinate resources
  • Collect, document, and maintain all member information and care management activities to ensure regulatory compliance
  • Provide education to members and families on disease processes, care gaps, healthcare instructions, and available benefits
  • May precept and support clinical new hires through onboarding, coaching, and competency development

What We're Looking For

  • Degree from an accredited school of nursing or Bachelor's degree in Nursing
  • Active RN license in North Carolina (NC RN license required)
  • 4–6 years of related nursing experience
  • Care management experience strongly preferred
  • Experience working with high-risk populations and complex care coordination
  • Strong assessment, critical thinking, and documentation skills
  • Ability to collaborate effectively with healthcare providers, members, and families
  • Knowledge of state and federal healthcare regulations
  • Excellent communication and education skills
  • Must reside in North Carolina
