Registered Nurse (Remote Patient Monitoring and Chronic Care Management)

Responsibilities

Our client, a confidential healthcare organization, is seeking a Registered Nurse to deliver remote patient monitoring and chronic care management services. In this fully remote role, you will connect with patients daily to support their chronic health needs from your home office.

Your day-to-day responsibilities include:

• Welcoming patients into continuous care programs and reviewing available benefits and services
• Coordinating with therapists and providers to ensure a collaborative approach to patient care
• Educating patients on the use of in-home monitoring devices when applicable
• Creating personalized, comprehensive care plans via phone or video visits
• Identifying and addressing barriers to patient success through regular engagement
• Providing specific education and coaching on chronic conditions
• Serving as the patient's first contact for non-emergent needs such as medication refills and appointment reminders
• Assisting patients with healthcare navigation, including scheduling appointments and coordinating with specialists
• Reviewing and evaluating device readings in real time during normal business hours
• Following protocols for identifying, communicating, and documenting trends and symptoms
• Escalating concerning readings or symptoms to providers according to established protocols
• Building meaningful rapport that fosters trust and motivates positive health changes

What We're Looking For

We are representing our client in the search for a compassionate, tech-savvy RN with the following qualifications:

• Active and unrestricted RN license in Georgia, Florida, Texas, or South Carolina (Compact License preferred)
• At least three years of experience in adult health preferred
• Background in adult chronic health conditions such as pulmonology, cardiology, or care management preferred
• Experience with a variety of Electronic Medical Record (EMR) systems
• Proficient knowledge and interest in basic computer skills
• Strong problem-solving abilities and capacity to multi-task effectively
• Excellent written and oral communication skills
• Demonstrated teamwork skills
• Clean background check and drug screening required
• Comfortable working remotely while maintaining collaborative relationships with a distributed team
