About the Role
This case manager position supports individuals in permanent supportive housing who have experienced chronic homelessness and face complex health, mental health, and substance use challenges. Working from our DTLA Flor Site, you'll engage with clients throughout their housing journey—from street outreach through stable permanent housing—using a comprehensive, client-centered approach that breaks cycles of homelessness and builds lasting community integration.
What You'll Do
- Conduct thorough intake interviews and individualized assessments to understand each client's housing, income, medical, mental health, substance use, vocational, and social support needs
- Develop personalized service plans that align with client-identified goals and PATH program standards
- Complete evidence-based assessments such as the VI-SPDAT to inform service delivery and housing placement decisions
- Connect clients with community resources, referrals, and supportive linkages that advance their long-term stability
- Monitor client progress, conduct structured 90-day reviews, and adjust service plans based on evolving needs and strengths
- Maintain accurate, timely case file documentation in compliance with HIPAA, program policies, and funder requirements
What We're Looking For

Requirements
- Valid driver's license, reliable personal transportation, proof of insurance, and ability to qualify for PATH vehicle coverage (driving is essential to this role)
- Employment authorization in the United States; PATH does not sponsor visas
- Willingness to obtain CPR/First Aid certification prior to start date
- Successful completion of tuberculosis screening, background check, and drug test as a condition of employment
