About the Role
As our Housing Navigator for the North Long Beach office (SPA 8), you'll use a "whatever it takes" philosophy to help clients move from homelessness to stable housing. You'll manage the full client journey—from initial assessment through 90-day check-ins and beyond—developing personalized support plans and connecting people with the resources they need. This role requires driving throughout Los Angeles County to meet clients at their locations and connect them to services in the community.
What You'll Do
- Conduct intake interviews and comprehensive needs assessments, gathering information on housing, income, health, mental health, substance use, financial resources, employment, and social support
- Develop individualized service plans with each client that identify housing goals, income stability, and personal objectives
- Assess housing vulnerability using validated assessment tools (VI-SPDAT) and other program-specific evaluations
- Identify and provide referrals to housing, employment, healthcare, mental health, and community-based resources
- Conduct 90-day progress reviews with clients, evaluating outcomes and adjusting support plans as needed
- Maintain accurate, complete case files and documentation in compliance with HIPAA regulations and program policies

- Sound judgment, integrity, and commitment to maintaining professional boundaries
- Preferred: Two years of relevant experience in case management, healthcare, homeless services, or supportive work with vulnerable populations
- Minimum: All education and experience levels are encouraged to apply
Mandatory Requirements
- Valid U.S. work authorization (we cannot sponsor visas)
- Valid driver's license and reliable transportation with current insurance
- CPR and First Aid certification (or agreement to complete before hire date)
- Pass tuberculosis test, background screening, and drug test as conditions of employment
