What You'll Do

• Conduct comprehensive patient assessments to identify clinical, psychosocial, and educational needs.
• Develop, implement, and update personalized care plans in collaboration with physicians and specialists.
• Facilitate transitions of care, including discharge planning, home health coordination, and specialist referrals.
• Provide ongoing patient and family education on disease management, medication adherence, and lifestyle modifications.
• Monitor patient progress, adjust interventions as needed, and document all activities accurately in the electronic health record.
• Serve as a liaison between patients, caregivers, and interdisciplinary team members to ensure clear communication and timely follow‑up.
• Participate in quality improvement initiatives and adhere to regulatory standards and organizational policies.

What We're Looking For

• Active, unrestricted RN license in the state of practice.
• BSN preferred; ADN with relevant experience considered.
• Minimum of two years of clinical nursing experience, preferably in case management, care coordination, or ambulatory settings.
• Strong assessment and critical‑thinking skills with the ability to prioritize competing demands.
• Excellent interpersonal and communication abilities for educating diverse patient populations.
• Proficiency with electronic medical records and standard office software.
• Commitment to delivering compassionate, patient‑centered care aligned with the organization’s mission.
