About the Role
You'll work nights at our 150-bed center providing skilled nursing and intermediate-level care to residents with diverse medical and cognitive needs. Our facility is known for evidence-based dementia care and behavioral health support—work that requires both clinical skill and genuine compassion for vulnerable populations.
What You'll Do
- Perform direct patient care including vital signs monitoring, personal hygiene assistance, and wound management
- Administer medications and treatments under physician direction while maintaining accurate medical records
- Monitor patient condition and report changes to nursing staff and physicians immediately
- Support residents with memory loss and behavioral challenges using trauma-informed and person-centered approaches
- Collaborate with the interdisciplinary care team to develop and execute individualized care plans
- Maintain a clean, safe, and therapeutic environment for all residents
What We're Looking For
- Current, valid Licensed Practical Nurse (LPN) license in good standing
- Proven experience in skilled nursing, rehabilitation, or long-term care settings
- Experience or strong interest in caring for residents with dementia or behavioral health needs
- Strong assessment and communication skills with patients, families, and healthcare teams
- Ability to work independently and manage priorities during night shifts
- Commitment to patient dignity, safety, and quality of life
