• Formulate and carry out evidence-based treatment plans that may include medication prescriptions, wound management, and minor procedures typical of urgent care.
• Deliver clear education to patients and caregivers regarding diagnoses, self-care steps, medication use, and recommended follow-up resources.
• Complete thorough, timely documentation in the electronic health record to support continuity and regulatory compliance.

What You Bring

• Active unrestricted licensure as a Nurse Practitioner or Physician Assistant eligible to practice in Connecticut.
