Role Overview
As a Community Coach 1, you'll work directly with service recipients in a full-time capacity, reporting to the Community Based Services Supervisor. This position offers the opportunity to make meaningful change by supporting individuals as they navigate daily living, pursue personal goals, and build meaningful connections within their local community. You'll approach each interaction with professionalism, respect, and a genuine commitment to helping people build on their own strengths and preferences.
Key Responsibilities
- Deliver individualized coaching and support to help service recipients achieve independence and self-direction in their home environments
- Foster community engagement by helping individuals identify and pursue opportunities for meaningful participation and inclusion
- Build trusting relationships based on the recipient's needs, values, and aspirations
- Collaborate with the Community Based Services Supervisor and broader team to coordinate services and monitor progress
- Maintain accurate documentation and complete required reporting related to services provided
- Respond with flexibility and creativity to support individuals in overcoming obstacles to independence and community connection
