RN (Registered Nurse) Case Manager, Home Health

Phoenix Home Care and Hospice is seeking a compassionate and detail-oriented Registered Nurse (RN) Case Manager for our home health program serving Warsaw, Missouri, and the surrounding region. In this full-time position, you'll work directly with patients and families in their homes, coordinating personalized care plans that enhance quality of life and support independence.

About This Position

As an RN Case Manager in home health, you are the clinical leader and primary advocate for your patient population. You'll balance hands-on nursing assessment with care coordination, building strong relationships with patients, families, physicians, and multidisciplinary teams. This role offers the meaningful impact of home-based care—where your clinical decisions and compassionate presence directly shape patient outcomes and family confidence.

Day-to-Day Responsibilities

• Conduct skilled nursing assessments and home visits to evaluate patient clinical needs and home environments
• Develop, implement, and update individualized care plans aligned with physician orders and patient/family goals
• Monitor patient progress, vital signs, and clinical changes; communicate findings to the care team and physicians
• Manage patient education on medications, disease processes, and self-care strategies
• Coordinate care with physicians, therapists, social workers, and other providers to ensure comprehensive support
• Advocate for patient needs and address barriers to care within the home setting
• Maintain accurate, timely clinical documentation and ensure compliance with regulatory standards
• Respond to patient concerns and adjust care plans as clinical conditions evolve

Requirements

• Current RN license (active, unrestricted) in Missouri or ability to obtain licensure
• Clinical nursing experience; home health or community-based care experience is preferred
• Strong clinical assessment and critical-thinking skills
• Excellent communication and interpersonal abilities with patients, families, and multidisciplinary teams
• Ability to work independently in varied home environments and manage time effectively across a patient caseload
• Reliable transportation and a valid driver's license
• Commitment to patient-centered, compassionate care
