Phoenix Home Care and Hospice is seeking a compassionate and detail-oriented Registered Nurse (RN) Case Manager for our home health program serving Warsaw, Missouri, and the surrounding region. In this full-time position, you'll work directly with patients and families in their homes, coordinating personalized care plans that enhance quality of life and support independence. About This Position As an RN Case Manager in home health, you are the clinical leader and primary advocate for your patient population. You'll balance hands-on nursing assessment with care coordination, building strong relationships with patients, families, physicians, and multidisciplinary teams.
