Medical Social Worker – PRN

Amedisys is seeking a Medical Social Worker – PRN to serve the Phoenix, Maricopa County area, providing essential psychosocial support to patients in their homes.

About the Role

This PRN position offers flexible scheduling while delivering vital social work services across the East Valley territory. You will travel to client residences, conduct assessments, and collaborate with healthcare teams to ensure comprehensive care plans are followed.

What You'll Do

• Visit patients in their homes to evaluate emotional, social, and environmental factors affecting health.
• Develop and adjust discharge and care plans in coordination with nurses, therapists, and physicians.
• Provide counseling and crisis intervention for patients and families facing illness, disability, or end‑of‑life issues.
• Connect individuals with community resources such as financial assistance, housing support, and caregiver services.
• Maintain accurate, timely documentation in electronic health records according to agency and regulatory standards.
• Participate in interdisciplinary meetings to communicate patient needs and advocate for appropriate services.
• Adhere to all state and federal regulations governing social work practice in healthcare settings.

What We're Looking For

• A Master’s or Doctorate degree in Social Work from a program accredited by the Council on Social Work Education (CSWE).
• Current licensure as an LCSW or LMSW in the state of Arizona.
• Minimum of one year of experience delivering social work services in a healthcare environment, such as a hospital, home health agency, or hospice.
• Demonstrated ability to conduct home‑based assessments and develop effective intervention plans.
• Strong interpersonal and communication skills, with sensitivity to diverse cultural and socioeconomic backgrounds.
• Reliable transportation and willingness to travel throughout the Phoenix Metro area as needed.
• Commitment to maintaining confidentiality and adhering to professional ethical standards.
