Manager, Care Coaching

Our recruiting team at Get A Job.ai is partnering with a confidential healthcare organization to fill a Manager, Care Coaching role. This remote position offers leadership responsibilities in a long-term care program serving two counties in Wisconsin. You'll mentor interdisciplinary teams delivering person-centered services to members with disabilities and frail elders.

This is a Monday–Friday position with primary remote work and occasional travel (at least once monthly) to team sites. You'll directly manage approximately 11–15 field staff while ensuring program quality and compliance.

Responsibilities

• Mentor and support Field Nurses and Care Coaches to ensure benefit delivery meets contractual standards
• Identify learning needs and facilitate orientation, training, and ongoing professional development for team members
• Assess and coach staff on core competencies and performance standards
• Review reports, documentation, and care management processes for quality and utilization
• Collaborate across departments to maintain provider service quality and effectiveness
• Participate in hiring, selecting, orienting, and training new associates
• Facilitate in-person unit meetings and attend quality visits as needed
• Educate community partners on program services

What We're Looking For

Required Qualifications:

• Bachelor's degree in human services or related field, OR Associate degree in nursing
• 1+ years of leadership experience in human services
• 3+ years of experience in Family Care programs
• Must reside within 1 hour of either Black River Falls or Blair, Wisconsin
• Reliable home internet (minimum 25 Mbps download, 10 Mbps upload)
• Dedicated workspace for handling protected health information

Preferred Qualifications:

• 5+ years working with individuals with intellectual/developmental disabilities, physical disabilities, or frail elders

Schedule: Monday–Friday, 8:00 AM – 4:30 PM CST (40 hours/week)

Pay
