About the Role
As a Licensed Practical Nurse with Bluebird Home Health, you'll provide direct patient care under the guidance of a registered nurse. Following each patient's individualized plan of care, you'll administer treatments and partner closely with the broader care team to ensure every patient receives safe, attentive, and high-quality home health services.
Key Responsibilities
- Deliver direct, hands-on patient care in accordance with the established plan of care
- Administer prescribed treatments and monitor patient progress under registered nurse supervision
- Collaborate effectively with members of the interdisciplinary care team to coordinate patient needs
- Document care accurately and communicate any changes in patient condition to the supervising RN
- Uphold professional nursing standards while supporting patients in a home-based setting
Qualifications
- Current, valid Licensed Practical Nurse (LPN) license in good standing
- Ability to follow a patient plan of care and work under the supervision of a registered nurse
- Strong collaboration and communication skills for working within a care team
- Commitment to compassionate, patient-centered home health care
- Availability for full-time work within the US
