About the Role
You'll deliver skilled nursing care to dually diagnosed Medicare beneficiaries in their home environments and at provider offices throughout Baltimore City, Baltimore County, and a 50-mile service radius. In this community health role, you'll develop meaningful patient relationships, manage your own caseload with clinical autonomy, and see an average of 2–3 patients daily, allowing you to provide thorough, personalized attention to each person you serve.
What You'll Do
- Assess patients and develop individualized care plans addressing multiple chronic conditions
- Perform skilled nursing assessments, interventions, and health screenings in home and office settings
- Coordinate care with physicians, social workers, and interdisciplinary team members
- Manage patient medications, monitor treatment compliance, and track disease progression
- Educate patients and families on health conditions, disease management, and preventive care
- Maintain comprehensive, compliant patient documentation and medical records
What We're Looking For
- Current Registered Nurse licensure in Maryland or compact state license
- Experience in home care, community health, or primary care nursing
- Excellent patient communication and education skills
- Ability to work independently and manage your own patient load
- Valid driver's license and reliable transportation
- Dedication to quality patient-centered care
