Role Overview

As a Home Health Physical Therapist, you will play a crucial role in helping patients recuperate from illness or injury in the comfort of their own homes.

Key Responsibilities

• Evaluate and treat patients with various physical impairments
• Create personalized treatment plans to improve patient mobility and function
• Maintain accurate medical records and communicate progress to interdisciplinary team members
• Collaborate with other healthcare professionals to ensure comprehensive care for each patient
• Provide education and support to patients and their families on home-based rehabilitation techniques

Requirements

• Licensed Physical Therapist (PT) in the state of practice
• Minimum 1-2 years of experience in home health or similar settings
• Strong clinical skills and ability to work independently
• Excellent communication and interpersonal skills
• Proficiency with electronic medical record systems
