BAYADA Home Health Care is hiring a full-time Weekend Baylor Physical Therapist, PT, Home Health Visits based in Warminster, PA 18974 to deliver visit-based care for adult and geriatric clients across Bucks County and Northeast Philadelphia.

The Opportunity

This full-time Weekend Baylor Physical Therapist, PT, Home Health Visits position centers on weekend coverage with available options of Friday through Sunday or Saturday through Monday. You will serve clients in designated areas near Warminster, PA while working independently in their homes. Previous home health experience is required to succeed in supporting recovery and independence for the adults and older adults this team assists.

Responsibilities

• Travel to client residences within assigned local territories to provide on-site care.
• Conduct evaluations that measure joint mobility and range, transfer ability, stability, walking patterns and form, muscle strength and stamina, balance, and overall safety.
• Apply evidence-supported protective measures for the skin and soft tissue that deter wound formation and encourage healing through appropriate dressings and agents.
• Design and deliver personalized treatment approaches that can include manual therapeutic exercises, gait practice, balance activities, and related techniques.
• Monitor progress continually, update the physical therapy plan as needed, and contribute to coordinated multidisciplinary plans involving nursing colleagues when suitable.
• Instruct clients along with family members or other representatives on rehabilitative activities that foster health, safety, and greater independence.
• Document all observations, services rendered, and care evaluations accurately using a dedicated tablet device.

Qualifications

• Active Physical Therapist license issued by the state of Pennsylvania.
• Minimum of one year of recent professional experience working as a Physical Therapist.
• Graduation from an educational program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the American Medical Association, demonstrated via transcript or diploma.
• Proven ability to operate independently while organizing time productively.
• Strong capabilities in interpersonal communication, task organization, and problem resolution.
• Reliable computer proficiency, with prior electronic medical records experience preferred.
