Position Summary

Your Responsibilities

• Carry out an individualized care plan matched to every patient’s unique needs
• Travel to clients’ homes within assigned local territories around Norristown, PA
• Complete ordered tasks including medication administration, wound management, treatments, and clinical procedures
• Track each client’s condition and promptly inform the Clinical or Client Services Manager of any shifts
• Review, implement, and carefully record all physician instructions
• Conduct client evaluations whenever they are indicated
• Oversee case coordination and multidisciplinary communication
• Log observations, interventions, and outcomes thoroughly on a modern tablet device

Requirements

• Active Registered Nurse license valid in Pennsylvania
• At least one year of recent, verifiable medical/surgical clinical experience
• Earlier home care work is preferred yet not mandatory
• Graduation from an accredited nursing program confirmed by diploma or official transcript
• Capacity to function independently while managing time productively
• Well-developed interpersonal abilities
• Competent computer skills; electronic medical record experience is advantageous
• Willingness to travel to scheduled client locations
