About This Position

We are seeking a dedicated and experienced Occupational Therapist (OT) for part-time employment to join our team. As an OT, you will utilize your clinical expertise to assess, teach, and deliver therapy interventions and treatment directed by the Physician as established in the home health plan of care. You will collaborate with interdisciplinary team members, receiving and providing pertinent updates to optimize patient well-being.

Key Responsibilities

• Assess patients' needs and develop individualized treatment plans
• Provide therapy interventions to improve daily living skills and functional independence
• Collaborate with interdisciplinary team members, including physicians, nurses, and social workers
• Document patient progress and communicate updates to the care team
• Ensure compliance with all regulatory requirements and standards of practice

Qualifications

• Master's degree in Occupational Therapy from an accredited program
• Licensed as an OT in the state of employment
• Minimum 1 year of experience in home health or related field
• Strong clinical skills and ability to work independently
• Excellent communication, collaboration, and documentation abilities
