Family Service Advocate

National Youth Advocate Program is hiring a full-time Family Service Advocate for our Care Management Team in Grandview Heights, Franklin County. In this role, you'll support families as they navigate challenges, access resources, and build stronger connections within their communities.

What You'll Do

• Provide direct advocacy support to families enrolled in our care management program
• Connect families with community resources and services that meet their identified needs
• Build trusting relationships with family members to understand their strengths and challenges
• Develop and implement comprehensive family support plans in collaboration with team members
• Document services provided and monitor progress toward family-identified goals
• Participate in case conferences and team meetings to coordinate services and ensure continuity of care
• Maintain accurate records and communicate effectively with families and colleagues
