PATH (People Assisting the Homeless) is hiring a dedicated CTI Case Manager for our Supportive Housing Services division in Los Angeles County. This is a full-time position where you'll directly impact housing stability and independence for individuals with a history of homelessness and complex support needs.

About the Role

As a Critical Time Intervention (CTI) Case Manager, you'll provide intensive, individualized case management to residents living in our Permanent Supportive Housing developments across Los Angeles County. Your role centers on helping clients maintain their housing, navigate complex systems, and build the skills and connections needed for long-term stability and community integration. You'll work both independently and alongside a multidisciplinary team to ensure clients have access to the resources, services, and support required to thrive.

Core Responsibilities

  • Maintain regular contact with residents to deliver strengths-based case management, including guidance on self-sufficiency, job readiness, and community resource navigation
  • Co-develop individualized housing stability and life goals plans with each resident, then track progress and adjust approaches as needed
  • Assess risk factors and respond to crises, helping clients build their own capacity to handle challenges, make sound decisions, and manage their health and independence
  • Act as an advocate and liaison between residents and service providers, landlords, healthcare systems, and benefits agencies to secure housing, medical services, transportation, and other stabilizing supports
  • Deliver in-home and community-based services, meeting clients in their living environments and neighborhoods

  • A valid driver's license and reliable transportation (driving is essential to this role)
  • Flexibility to work variable hours, including weekends and evenings
  • Openness to obtaining CPR and First Aid certification if not already held
  • Two years of case management, healthcare, homeless services, or similar work experience preferred; all backgrounds are welcome

Required Before Your Start Date

  • U.S. work authorization
  • Current driver's license and proof of vehicle insurance and ownership for work use
  • Ability to qualify for PATH vehicle insurance coverage
  • Full COVID-19 vaccination
  • Tuberculosis test, comprehensive background screening, and drug screening

Compensation

Full-time, non-exempt position. Pay range: $23.24–$28.28 per hour, distributed across 24 pay periods annually. Candidates meeting minimum qualifications begin at the lower range; those with substantial relevant experience may start at the higher range.
