ADON Health and Wellness Coordinator

About the Role

This ADON Health and Wellness Coordinator position bridges clinical care and resident wellbeing, working directly with our nursing leadership to create and deliver holistic health services and wellness programs. You'll play a vital role in ensuring every resident—whether independent or memory care—receives compassionate, person-centered support tailored to their health goals and individual needs.

What You'll Do

• Partner with the Assistant Director of Nursing to assess resident health needs and develop individualized wellness plans
• Coordinate health education, fitness activities, nutrition programs, and cognitive wellness initiatives for all residents
• Support medication management protocols and health monitoring while maintaining accurate documentation
• Facilitate communication between residents, families, and healthcare providers to ensure coordinated care
• Promote preventive health practices and encourage active, engaged lifestyles among residents
• Uphold health and safety compliance standards while modeling compassion and dignity in all interactions

What We're Looking For

• Nursing certification or healthcare background (CNA, LPN, RN, or equivalent)
• Genuine passion for senior health, dignity, and quality of life
• Strong communication skills and ability to connect meaningfully with residents and families
• Experience working collaboratively within a care team environment
• High school diploma or GED; some college or healthcare coursework preferred
