Phoenix Home Care and Hospice is seeking dedicated caregivers and certified nursing assistants to join our team in Liberal, Kansas. This is a full-time position serving families and patients throughout the community who depend on compassionate, professional care.

The Opportunity

We're building a care team that puts patients and their families first. As a caregiver or CNA with Phoenix Home Care and Hospice, you'll work directly in patients' homes, providing essential support and building meaningful relationships with the people you serve. Key Responsibilities

• Provide personal care assistance including bathing, dressing, grooming, and toileting
• Support patients with mobility, transferring, and positioning to prevent injury
• Monitor and report changes in patient condition to the clinical team
• Assist with light housekeeping and meal preparation as part of patient care
• Maintain accurate patient records and communicate clearly with supervisors
• Follow established care plans and respond to patient needs with professionalism and compassion

• Current CNA certification or equivalent experience in patient care
