Complex Discharge Planner

About This Opportunity

We are representing a confidential healthcare organization seeking a Complex Discharge Planner to join their care coordination team. This fully remote role focuses on developing and managing discharge plans for high-risk members transitioning between levels of care.

Our client values clinical expertise in discharge planning and is looking for an experienced RN who can work independently in a telephonic environment while coordinating with providers, facilities, and community resources.

Responsibilities

• Develop complex discharge plans based on member needs assessments to ensure appropriate level of care
• Coordinate with providers, community agencies, and utilization review staff to facilitate safe and clinically appropriate transitions
• Manage member discharge plans to ensure timely transitions between levels of care and facilities
• Document and maintain all discharge plans in health management systems according to established policies and guidelines
• Educate members and families on discharge plans and utilization processes
• Provide feedback to leadership on process improvement opportunities for discharge planning
• Coordinate prior authorizations related to discharge planning as needed
• Support concurrent review team activities when required

What We're Looking For

• Graduate from an accredited school of nursing or Bachelor's degree in Nursing required
• Active Compact RN License (multi-state) required
• 2–4 years of related nursing experience required
• 2+ years of acute care experience required
• Prior case management and/or discharge planning experience in a telephonic environment highly preferred
• Clinical knowledge of discharge planning competencies preferred
• Knowledge of Medicare and Medicaid regulations preferred
• Knowledge of utilization management processes preferred
• Strong communication and coordination skills for working with providers, facilities, and families
