Fira-health is hiring a full-time Physical Therapist (PT) to deliver skilled home health care across Collin County. This field-based position runs Monday through Friday with occasional weekend availability, where you'll manage complex cases and drive measurable patient outcomes in patients' homes.
The Opportunity
As a Physical Therapist at Fira-health, you'll lead your own caseload and shape the direction of patient care from assessment through discharge. You'll work with clinical autonomy to develop treatment plans, make real decisions about patient progression, and measure outcomes that matter—how patients move, function, and maintain safety at home. Our operational team and purpose-built technology remove administrative friction so you can concentrate on skilled clinical work.
Your Responsibilities
- Perform comprehensive in-home assessments and deliver skilled physical therapy visits responsive to each patient's medical history and living environment.
- Design and implement individualized care plans addressing gait mechanics, balance training, transfers, fall prevention, strengthening, and functional mobility restoration.
- Establish measurable therapy goals, track objective progress data, and modify treatment when patients plateau or conditions shift.
- Lead multidisciplinary coordination—reporting clinical changes to nursing and physician partners, aligning therapy orders, and keeping the care team aligned.
- Teach patients and caregivers practical home exercise routines and home safety adjustments that stick after therapy ends.
- Supervise physical therapy assistants (PTAs) and oversee their contributions to patient care plans as appropriate.
What We're Looking For
- Active Texas PT license in good standing.
- Strong independent clinical judgment and comfort managing complex patients in field-based settings.
- Competence recognizing clinical changes, escalating appropriately to the medical team, and documenting clinical reasoning transparently.
- Core expertise in home health physical therapy including transfers, gait training, balance and fall prevention, assistive device fitting, and functional mobility assessment.
- Clear communication skills across patients, families, and interdisciplinary clinicians.
- Valid driver's license, reliable transportation, and willingness to travel throughout your service area and transport basic therapy equipment.
- Understanding of productivity-based compensation ($75 per productivity point; target 30 points weekly, yielding $100,000–$130,000 annually).
- Home health background, experience with high-acuity patient populations, or PTA supervision experience are strong differentiators.
