Case Manager – Vets

Axess Family Services seeks a dedicated Case Manager to serve veterans in our Lorain office. This full-time position offers the opportunity to make a meaningful difference in the lives of veterans and their families by providing compassionate, evidence-based case management services.

Role Overview

As a Case Manager serving veterans, you'll work closely with our Assistant Director and site Program Manager to provide comprehensive support to clients navigating complex personal and social challenges. You'll apply trauma-informed care principles in all interactions, helping veterans access the resources and stability they need to build healthier lives. This role is central to our mission of strengthening families and communities in Lorain County.

Your Responsibilities

• Conduct thorough client assessments to identify needs, strengths, and barriers to well-being
• Develop and implement individualized case plans aligned with client goals and organizational standards
• Connect veterans with community resources, social services, mental health support, employment opportunities, and housing assistance
• Document client interactions and progress through detailed, accurate case notes
• Maintain regular contact with clients to monitor progress and adjust support as needed
• Collaborate with interdisciplinary team members, community partners, and family members to coordinate care
• Uphold all Axess Family Services policies, procedures, and professional standards

What You Bring

• High school diploma or GED (associate degree or higher preferred)
• Experience in case management, social services, counseling, or related field
• Strong written and verbal communication skills
• Demonstrated ability to work compassionately with individuals who have experienced trauma
• Excellent organizational and time-management skills
• Ability to maintain detailed records and documentation
• Commitment to evidence-based, trauma-informed practice
