PATH (People Assisting the Homeless) is seeking a full-time Outreach Case Manager for the Metro Rail Outreach Team in Los Angeles County. You'll provide frontline outreach and case management to people experiencing homelessness within the Metro transit system, helping approximately 23 individuals per week connect to stable housing and supportive services.

What You'll Do

  • Conduct field-based outreach across Metro Los Angeles to engage people experiencing homelessness and build trusting relationships.
  • Administer the Vulnerability Index-Service Prioritization Decisions Assistance Tool (VI-SPDAT) to assess client needs and identify appropriate services.
  • Develop and review Individualized Service Plans (ISP) collaboratively with clients to establish housing and support goals.
  • Maintain regular contact with clients to track progress, overcome obstacles, and support achievement of identified goals.
  • Support clients with housing applications, benefit program enrollment (General Relief, SSI, TANF, Veterans Pension), employment referrals, and documentation gathering.
  • Connect clients to healthcare, mental health services, and substance abuse treatment resources throughout the community.
  • Advocate on behalf of clients with landlords, social service agencies, and community partners.
  • Meet contracted response timeframes for service requests and maintain thorough client documentation and records.

What We're Looking For

  • Minimum two years of experience in outreach, case management, or related work supporting homeless or underserved populations.
  • Strong written and verbal communication skills to engage people from diverse backgrounds clearly and effectively.
  • Analytical mindset with careful attention to detail; ability to identify patterns and inform program decisions.
  • Capacity to work both independently and collaboratively with colleagues, clients, and community partners.
  • Creative problem-solving ability and willingness to develop innovative approaches to addressing barriers.
  • Professional ethics, personal integrity, and commitment to maintaining appropriate boundaries with clients.
  • Valid driver's license, reliable personal vehicle, proof of insurance, and ownership documentation.
  • Flexibility to work variable shifts including weekends, with schedules subject to change based on operational needs.

  • Proof of U.S. employment authorization.
  • CPR and First Aid certification, or willingness and ability to obtain before hire date.
  • Successful completion of tuberculosis screening, background check, and drug screening as employment conditions.
  • Ability to qualify for PATH's required vehicle insurance coverage.
