About the Role

As a Case Manager with PATH's Time Limited Subsidy program, you'll serve individuals across all phases of homelessness—from street-based situations to permanent housing. Working Monday through Friday, 8:00 AM to 4:30 PM, in Long Beach, CA 90805, you'll provide intensive individualized support using a client-centered, strengths-based approach. This full-time, hourly position requires reliable personal transportation and a valid driver's license, as driving is essential to conducting home visits, attending community meetings, and coordinating services throughout Los Angeles County.

What You'll Do

  • Conduct comprehensive intake and assessment meetings with clients; develop individualized service plans (ISP) addressing housing goals, income, health, mental health, substance use, employment, and personal aspirations
  • Complete structured assessments (e.g., VI-SPDAT) to guide prioritization and resource matching
  • Identify, refer, and link clients to healthcare, mental health, social services, and community resources; follow up to ensure engagement and progress
  • Conduct 90-day reviews to assess progress, celebrate wins, adjust goals, and sustain client momentum
  • Provide crisis intervention and safety planning; equip clients with coping strategies and independent problem-solving skills
  • Apply trauma-informed and motivational interviewing approaches in all client interactions
  • Maintain timely, complete, accurate case documentation in compliance with HIPAA and organizational standards
  • Cultivate strong working relationships with colleagues, clients, and partner organizations to coordinate wrap-around care

You must also have reliable personal transportation, a valid driver's license, proof of vehicle insurance and ownership, and meet PATH's insurance requirements.

Compensation and Benefits
