About the Role

As Team Manager, Hospice Home Care, you'll lead our Queens hospice team in delivering person-centered care aligned with VNS Health's commitment to dignity and comfort. You'll manage day-to-day operations, coordinate clinical care delivery, and ensure every patient and family receives seamless support across our services—including after-hours availability. Your leadership will drive quality improvement initiatives while maintaining full compliance with state and federal hospice regulations.

What You'll Do

  • Manage, supervise, and support the interdisciplinary hospice team, including nurses, social workers, spiritual care providers, and volunteers
  • Coordinate patient and family care planning, clinical assessments, and care adjustments in the home setting
  • Oversee implementation of clinical quality improvement and patient safety initiatives
  • Ensure regulatory compliance and adherence to hospice care standards
  • Provide clinical leadership and mentoring to team members
  • Support on-call responsibilities as needed to ensure continuity of care
