Join Hartford HealthCare Medical Group as a Nurse Practitioner (APRN) or Physician Assistant in Wolcott, New Haven County, providing primary care to our community at our location on Hart Street in New Britain. This is a full-time position Monday through Friday where you'll partner with physicians and care teams to deliver comprehensive, patient-centered healthcare.

About the Role

As a Nurse Practitioner or Physician Assistant, you'll manage the full spectrum of primary care—from acute illness and chronic disease management to preventive health and wellness promotion. You'll work in a collaborative environment where your clinical expertise directly impacts patient outcomes and community health.

What You'll Do

  • Conduct patient evaluations, physical exams, and diagnostic assessments
  • Diagnose and treat common acute and chronic conditions
  • Develop and implement individualized treatment plans
  • Order and interpret diagnostic tests and imaging
  • Prescribe medications and manage ongoing medication therapy
  • Counsel patients on lifestyle modifications, disease prevention, and health maintenance
  • Document clinical encounters and maintain detailed patient records
  • Coordinate care with specialists and other healthcare providers
  • Participate in quality improvement and clinical initiatives

What We're Looking For

  • Valid Connecticut APRN or Physician Assistant license
  • Current certification in your specialty (AANP, ANCC, NCCPA, or equivalent)
  • Strong clinical knowledge and decision-making skills
  • Excellent communication and patient education abilities
  • Commitment to evidence-based practice and continuous learning
  • Ability to work effectively within a team-based care model
