About the Role
As the Social Service Coordinator for Mount Vernon Manor I & II, located at 3307 Wallace Street in Philadelphia, you will help individuals and families access community services and programs that improve daily life. Working within the Impact and Service Delivery department, you will report to the Regional Lead SSC and the Regional or Senior Associate Director. This position follows a 31-hour weekly schedule: Monday, Thursday, and Friday from 9:00 a.m. to 3:00 p.m., and Tuesday and Wednesday from 10:00 a.m. to 5:00 p.m., each including a 30-minute lunch. The pay range is $21.00 to $23.00 per hour.
Key Responsibilities
- Coordinate and facilitate supportive services for residents and families at Mount Vernon Manor I & II.
- Build relationships with residents to understand their needs and connect them to appropriate local resources.
- Partner with community organizations to expand access to programs and assistance.
- Maintain accurate records and documentation related to service delivery.
- Collaborate with the Regional Lead SSC and leadership team to advance program goals.
