As a Family Counselor – Case Worker with Brooklyn Transitions, you join a specialized, borough-wide prevention initiative that supports households across Brooklyn navigating substance use and/or mental health concerns. Based at 503 5th Avenue in Brooklyn, NY, this on-site position reports to the Social Work Supervisor and follows a 35-hour, non-exempt schedule. The annual salary ranges from $50,000 to $55,000, determined by relevant post-graduate experience.
Key Responsibilities
- Deliver compassionate, family-centered case management to participants enrolled in the Brooklyn Transitions prevention program.
- Assess each family's strengths and needs, then develop and maintain individualized service plans.
- Connect families to community resources addressing substance use, mental health, and related supports.
- Conduct regular home and field visits across Brooklyn to monitor progress and reinforce stability.
- Maintain accurate, timely documentation in line with program and regulatory standards.
- Collaborate closely with the Social Work Supervisor and the broader care team to coordinate effective interventions.
