About the Role
This travel position places a skilled Medical Social Worker in Joshua Tree, San Bernardino County for a 13-week contract on a day schedule of five 8-hour shifts. You will support patients and families navigating medical challenges, connecting them with the resources and care coordination they need throughout their treatment journey.
Key Responsibilities
- Assess the psychosocial, emotional, and practical needs of patients and their families.
- Develop and coordinate care plans that link patients to community services, financial assistance, and follow-up support.
- Provide counseling and crisis intervention to individuals adjusting to illness, treatment, or discharge.
- Partner with physicians, nurses, and the multidisciplinary care team to ensure smooth transitions of care.
- Maintain accurate, timely documentation in accordance with facility and regulatory standards.
Qualifications
- Credentials and licensure required to practice as a Medical Social Worker in California.
- Background in healthcare, hospital, or clinical social work settings.
- Availability to begin as soon as possible and commit to the full 13-week assignment.
- Flexibility and adaptability suited to travel-based contract work.
