Street Health Outreach Worker

St. Vincent De Paul Village seeks a compassionate and dedicated Street Health Outreach Worker to join our team in San Diego, San Diego County. In this full-time position, you'll provide critical outreach, engagement, and care coordination services to individuals experiencing homelessness throughout the county, connecting them with resources and pathways toward sustainable housing, improved health, and stability.

About the Role

As a Street Health Outreach Worker, you'll serve as a vital link between vulnerable populations and the support systems designed to help them. Working across San Diego County, you'll engage individuals experiencing homelessness with compassion and consistency, building trust and facilitating access to essential services that lead toward housing stability and wellbeing.

What You'll Do

• Conduct street-level outreach in communities throughout San Diego County to connect with unhoused individuals
• Build trusting relationships and conduct needs assessments to understand barriers to housing and health care
• Coordinate care and facilitate connections to shelter, medical services, mental health resources, and social benefits
• Document client interactions and progress within case management systems
• Advocate for clients' needs and collaborate with community partners and service providers
• Provide information and education about available programs and benefits
• Support individuals in taking concrete steps toward housing stability and self-sufficiency
