State of South Carolina is hiring a Social Worker to serve as a Lead Team Case Manager in Market, Richland County. This is a full-time, in-office position within the Community Long Term Care division, where you'll combine hands-on case management expertise with team leadership to support individuals accessing essential long-term care services.

In this lead case manager position, you'll balance direct social work practice with supervisory responsibilities. You'll oversee a case management team while delivering excellent support to vulnerable clients, helping them navigate complex systems and access the services they need. What You'll Do

• Lead, mentor, and support a team of case managers with clinical guidance and professional development
• Assess client needs comprehensively and develop individualized service plans
• Coordinate care across medical, social, and behavioral health services
• Monitor progress, adjust plans as needed, and ensure clients remain safe and connected to community
• Advocate for clients to access benefits, resources, and support services
• Maintain accurate records and comply with state and federal regulations
• Build relationships with healthcare providers, community partners, and family members
• Contribute to team meetings and support continuous quality improvement

What We're Looking For

• Bachelor's degree in social work or related field; master's degree preferred
• Active license as a social worker (LSW or LCSW)
• 3+ years of case management experience in healthcare, long-term care, or social services
• Leadership experience or demonstrated capacity to manage and develop team members
• Excellent communication skills and ability to connect with diverse client populations
• Knowledge of healthcare systems, benefits programs, and social services landscape
• Strong organizational skills and ability to balance multiple priorities
• Commitment to ethical practice and client-centered care
