About the Role

The Social Worker - Ambulatory II is an integral member of UNC Health Care's interdisciplinary healthcare team. This level II position combines clinical expertise with the independence and judgment needed to provide direct social work services across ambulatory care environments. You'll assess complex psychosocial needs, coordinate care pathways, and connect patients with critical resources—all while documenting clinical work and collaborating closely with physicians, nurses, and other providers to support whole-person care.

What You'll Do

• Conduct psychosocial assessments identifying barriers to care, including emotional, financial, and social factors affecting patient health
• Develop and coordinate discharge plans that ensure safe transitions and continuity of care
• Connect patients with community resources, financial assistance, and support services tailored to their needs
• Provide counseling and education to patients and families on managing health conditions and navigating healthcare systems
• Maintain detailed, accurate clinical documentation in the electronic health record
• Advocate for patient needs within complex healthcare and social systems
• Collaborate with the clinical team to address psychosocial dimensions of care

What We're Looking For

• Bachelor's degree in Social Work (BSW) or related field; Master's degree preferred
• Current North Carolina licensure as LCSW or LMSW (or eligibility)
• Clinical experience in healthcare or ambulatory settings
• Strong assessment, care coordination, and interpersonal skills
• Ability to work both independently and collaboratively within interdisciplinary teams
• Commitment to serving diverse populations with cultural humility and compassion
