Physical Therapist (PT) – Outpatient

About the Role

As an outpatient Physical Therapist with H2 Health, you'll work directly with patients to restore mobility, reduce pain, and enhance quality of life. You'll conduct comprehensive evaluations, design individualized treatment plans, and guide patients through evidence-based therapeutic interventions in our modern outpatient facility. Your expertise will help patients of all ages recover from injury, manage chronic conditions, and return to the activities they value most.

What You'll Do

• Perform detailed patient assessments and develop tailored rehabilitation plans based on clinical findings and patient goals
• Deliver hands-on and therapeutic exercises in an outpatient setting, adjusting intensity and techniques to match each patient's needs
• Monitor patient progress and modify treatment strategies to achieve measurable, meaningful outcomes
• Educate patients and families about injury prevention, home exercise programs, and self-management strategies
• Document clinical findings, treatment notes, and progress records in compliance with regulatory and quality standards
• Collaborate with physicians, healthcare team members, and other professionals to coordinate comprehensive, integrated care
• Contribute to a welcoming, professional clinic environment that supports excellent patient experiences and team excellence

What We're Looking For

• Current, unrestricted Physical Therapist license in North Carolina
• Doctor of Physical Therapy (DPT) degree or equivalent qualification
• Strong clinical assessment, diagnostic reasoning, and hands-on treatment skills
• Proven experience delivering patient-centered care in an outpatient clinic environment
• Clear communication skills and genuine passion for helping patients achieve their rehabilitation goals
