Outpatient Hand Therapist CHT

H2 Health is hiring a full-time Outpatient Hand Therapist (CHT) to join our outpatient clinic in Solms, Comal County, Texas. About the Role

As an Outpatient Hand Therapist with H2 Health, you will deliver individualized, hands-on care to patients recovering from upper-extremity injuries and conditions. This on-site position is based at our outpatient location at 3016 Independence Dr, Ste 201, New Braunfels, TX 78132, serving the Solms and greater Comal County community. Beyond treating diagnoses, you will form genuine connections, guide patients through their recovery journeys, and continue developing your own clinical expertise.

Key Responsibilities

• Assess, plan, and carry out personalized therapy programs for patients with hand and upper-extremity conditions
• Provide skilled outpatient treatment as a Certified Hand Therapist, drawing on your background as an OT or PT
• Track patient progress and adjust treatment goals to support meaningful, lasting outcomes
• Educate patients and their families on home programs, recovery expectations, and injury prevention
• Maintain accurate, timely clinical documentation in line with professional and regulatory standards
• Collaborate with a supportive multidisciplinary team to deliver coordinated, patient-centered care

Qualifications

• Current licensure as an Occupational Therapist (OT) or Physical Therapist (PT) in the state of Texas
• Certified Hand Therapist (CHT) credential
• Experience or strong interest in outpatient hand and upper-extremity rehabilitation
• A genuine commitment to relationship-driven, personalized patient care
• Strong communication skills and the ability to thrive in a collaborative clinic setting
