Physical Therapist – Long Term Care – Direct Hire

Lighthouse Medical Staffing is hiring a full-time Physical Therapist for our Long Term Care facility in Wolfeboro, Carroll County, New Hampshire. What You'll Do

• Conduct thorough physical therapy evaluations and functional assessments for long-term care residents
• Develop personalized, evidence-based treatment plans focused on restoring mobility and independence
• Deliver hands-on therapeutic interventions and rehabilitative services tailored to individual resident needs
• Monitor patient progress, adjust treatment protocols, and communicate outcomes to the care team
• Partner with nurses, occupational therapists, physicians, and other disciplines to coordinate integrated care
• Maintain comprehensive clinical documentation in compliance with regulatory standards and facility requirements
• Educate residents and families on therapeutic exercises, safety strategies, and functional goals

What We're Looking For

• Current, valid Physical Therapist license in New Hampshire
• Bachelor's degree or higher in Physical Therapy from an accredited institution
• Clinical experience with geriatric or long-term care patient populations
• Strong communication, documentation, and interpersonal skills
• Demonstrated commitment to resident-centered, compassionate care delivery
• Ability to thrive in a collaborative, team-based healthcare setting
