About the Role

As an Occupational Therapist at 21hhs, you'll work directly with patients in their homes to evaluate functional limitations and develop customized treatment plans that enhance independence and safety. You'll collaborate with case managers and interdisciplinary care teams to address complex needs—from home modifications to adaptive strategies for chronic conditions. Your work contributes to measurable outcomes we're proud of: hospital readmission rates consistently below 10%, well below the national average of over 15%.

What You'll Do

• Evaluate patients in their homes to assess physical limitations, cognitive changes, and environmental safety concerns
• Design and implement occupational therapy interventions focused on activities of daily living, mobility, and independence
• Provide specialized services including wheelchair assessment, seating and positioning, and home safety modifications
• Educate patients and family caregivers on pain management, fall prevention strategies, and management of chronic diseases and dementia
• Develop caregiver training programs to empower families and improve long-term outcomes
• Serve as a clinical advocate and active member of the interdisciplinary care team

Requirements

• Current occupational therapy license in California
• Bachelor's degree or higher in occupational therapy from an accredited program
• Current CPR and BLS certification
• Valid driver's license and reliable transportation (field-based work throughout Santa Clara County required)
• Proficiency with clinical documentation and electronic health records
• Strong communication skills and commitment to collaborative care
