Position Summary
This part-time nursing position focuses on weekend home health admissions across the Santa Clara County service area. Your clinical judgment during these critical first visits directly influences patient outcomes and care quality throughout their home health journey, from evaluating medical history and functional status to identifying environmental safety concerns and determining the specific interventions required.
Responsibilities
- Conduct comprehensive initial assessments of newly admitted patients in their home environments
- Evaluate patient medical history, current medications, functional abilities, and home safety factors
- Determine appropriate home care service levels and clinical interventions based on assessment findings
- Document assessment data and communicate recommendations to the interdisciplinary care team
- Build trust with patients and families while explaining home health services, care expectations, and treatment goals
- Coordinate with nursing supervisors and clinical staff to develop individualized care plans
Skills & Qualifications
- Active California Registered Nurse (RN) license
- Strong clinical assessment and diagnostic reasoning capabilities
- Excellent written and verbal communication skills with patients, families, and healthcare professionals
- Ability to manage time effectively while working independently in diverse home environments
